think i found the problem... was using an email acc with a 0 in it.

If the survey works fine it's either an invalid e-mail address or is in the spam folder.

Weird, because the script and site work 100%, try a different host.

A quick question that have been bugging me; won't people be able to see that the adress is wrong and realize it is a scam?

And could someone post their site so we can see how it really looks when it is finished, I know you have got the screenshots but still it would be nice to check it out.

The address is something all the phishing sites have problems with.
You just have to disguise it a little.
Do you check all the sites you visit to see if it's the real deal?

The people who do would not have fallen for this scam anyway.
Only ignorant people fall for it, and those do not check addresses.
