Ok im bored, so i thought i could use my in-depth knowledge to make up a FAQ guide to encounters in end game situations, eg Molten Core all the way to Naxxramas. So if you have any questions, be it a 20 man raid, 40 man raid, or maybe even a 5 man instance, post your question here, i'll try my best to answer it.

Reach Revered with Hydraxian Waterlords and you will get an Aqual Quintessence. You use this to douse the flame, and Majordomo spawns.

He varies, but after 20 runs he has been there 10% of the time.

Nice ninja post, but anyway, exactly what he said, but now they introduced an item called "Eternal Quintessence" which does the some thing, but isnt consumed when you use it. It has a 1 Hour cooldown. You get this when you reach revered.

Also there are flames to be doused at: Magmadar, Gehennas, Garr, Shazzrah, Golemagg Bridge , Sulfuron, Golemagg.

By doing this quest you also get the choice of a Melee or Caster Based fire resist ring

Though i was aiming at more end-game isntance scenarios so i cannot be sure, but Miner Johnson i would say roughly appears every 1 in 5, or every 1 in 6 instances. This is using data collected from thottbot.
