If you notice theres no loading screen between ports from outland to Silvermoon, which means its in the same instance. Who knows? maybe you can fly there!

It came to my conclusion... I heart Creating..

All outland world maps are in the same continent instance.

Yep. If you use mapviewer you can travel between them. Azuremyst is straight east of SMV, and Eversong/Ghostlands is to the north-east of Netherstorm (at least according to mapviewer).
I assume you fall in the abyss when you get to these areas since flying isn't allowed but until someone finds a way to get between them on a live server I personally won't count this as confirmed.

Azuremyst, Bloodmyst, Ghostlands, IoQ, Eversong Woods and Outland are in the same instance. You can't swim to Kalimdor from Azuremyst or Bloodmyst, because Kalimdor is not in Outland. You can't swim from Ghostlands to Eastern Kingdoms, because EK is not in Outland.
You can't swim to Azuremyst or Bloodmyst from Kalimdor. And you can't swim from EK to Ghostlands.
