I tried to use this program today and it didn't work flawlessly. I had to make some modifications...
(I use rev 3449 by the way)

First SQL Query Generated:
'Creature_names' field should be changed to just 'names'
Delete fields 'unk_init1 and unk_init2'

Second SQL Query Generated:
There program did not generate the field name 'attacktype' - Easy fix as well.

For those of you who don't know what to put in rank fields:

0 - Normal
1 - Elite
2 - Rare Elite
3 - World Boss
4 - Rare
